Reviewing Today Tonight’s insulation exclusive

As an example of “tabloid TV” the Today Tonight (TT) report broadcast on 17 February 2010 concerning children assisting workers to install insulation, was very good.  It probably benefited from my own appearances remaining brief.

The topicality of a story on the home insulation industry could not have been higher yesterday as a Senate inquiry into the Australian Government’s environment and job creation scheme held hearings in Melbourne.  TT led its show with the scandalous report.

The video of a young boy handling large bags of insulation on a roof is disturbing; the unprotected handling of the insulation material by the young boy is similar.  That the children were allowed to be on the roof by the homeowner and parents is a parental supervision issue and outside the scope of this blog.  That the workers allowed them to be present and did not tell the children to get down is more disturbing and a clear breach of the workers’ OHS obligations. Don’t get sidetracked by depression marketing

Over the last couple of months, SafetyAtWorkBlog has written several articles on the psychosocial workplace hazard of depression, stress and anxiety.

Science Friday is a regular feature of the NPR  program, Talk of the Nation in the United States.  Last Friday, it focussed on depression.  Its speakers talked about how the diagnosis of depression has changed over the decades, sometimes to match the range of depression medications available.

Importantly there is a differentiation between depression and mental health. (Psychosocial disorders doesn’t seem to be a term used outside of OHS)  Depression is slowly becoming the collective term for sad, melancholy, unhappy, miserable, anxious……….  It is very important for workplace safety professionals to try to pierce the fug of depression marketing so that one is not distracted into the trap of treating workers for a personal problem rather than preventing the hazard through changing organisational attitudes. Okay, I don’t smell but am I safe?

King Gee recently released a range of work clothing that is manufactured using a technique that reduces the wearer’s body odour.   A sample was sent to SafetyAtWorkBlog unrequested.   For those tradespeople with a body odour issue, the clothing may be a godsend, maybe more so for the people they have to work with.   The new clothing has received at least one media mention.

The issue that has stopped me from wearing the sample shirt is that the “odour-killing” properties are due to a process of:

“…. engineering molecules at the nanoscale …[that] transforms the very fibers of the fabric to provide unsurpassed odour elimination.”

Nanotechnology is a recent technology that is being applied widely but without a detailed consideration of the possible health effects to the user, the environment and to those who manufacture nano-materials. Continue reading “Okay, I don’t smell but am I safe?”

Stress management may be only a chocolate cake away

Safety professionals often struggle to manage stress in their employees and themselves but new research has found links between the consumption of chocolate and a reduction in stress.

Update on New Zealand cool store explosion

Cabinet-making compliance

In March 2008, in Western Australia, a 22-year-old worker was crushed to death when a stack of veneered chipboard sheets toppled onto him.  This sparked an audit campaign of the cabinet-making industry in 2009 by WorkSafe WA about which some results were released on 12 January 2010.

Such results are not often covered in this blog but the number of improvement notices provide a useful summary of the persistent hazards present in this industry and on machinery that is used in a variety of workplaces. Continue reading “Cabinet-making compliance”

Unique company response to confined space penalty

In 2007, according to the ABC news site,

“42-year-old Geoffrey Johnson [died after he] inhaled toxic fumes from paint stripper when he was cleaning the inside of a large chemical tank”.

On 16 December 2009, his employer, Depot Vic P/L, was fined half a million dollars over this breach of the OHS legislation.

Initial reports say that the company is no longer in business but it

“told the court is had put aside money to pay the fine.”

Wow.  What happened to phoenix companies?  – the business scourge that closes down to avoid paying outstanding debts and, often the costs associated with a worker’s death, and then starts up again under a different structure.

That a company will pay a fine for an OHS breach years after ceasing business seems a remarkable and admirable act.

Hyde Park Tank Depot’s assets were purchased by the Scott Corporation several months after Mr Johnson’s death, according to information SafetyAtWorkBlog obtained from Scott Corporation.  The current business and website listing was not operating at the time of Mr Johnson’s death.

WorkSafe Victoria provided background to Mr Johnson’s death in a prosecution summary in April 2009.  The full summary gives a clear indication why the fine was so high.

“Depot Vic Pty Limited (formerly known as Hyde Park Tank Depot Pty Ltd) undertakes cleaning, repair and maintenance of ISO containers for the chemical industry.  ISO containers are confined spaces, being portable tanks used to transport chemicals.  The tanks are usually cleaned purely by hydro-blasting, but on occasion the tanks were required to be cleaned more thoroughly.

The system of work was such that when this situation occurred, the cleaning of the tank required 2 stages. The first stage involved the application of a cleaning agent, usually a product known as ‘Selleys Renovators Choice’ stripper (which is not a dangerous good).

The second stage then involved the use of hydro-blasting on the internal walls to remove the stripper and clean the wall.  The company’s work instructions required that a confined space permit be issued and that appropriate PPE be worn.

On 16 August 2007, an employee of Depot Vic Pty Limited died whilst attempting to remove latex from the internal walls of a 25,500 litre ISO tank.  The deceased had entered the tank and instead of using the ‘Selleys Renovators Choice’ stripper, had used a product known as ‘Paint Stripper Gel GS 125’ that was suited to clean external components only (and not the inside of the tank).  The label of this product contained safety directions such as “do not breathe vapour” and “use only in a well ventilated area”.  This product is a dangerous good ‘class 6.1 (toxic substance) of packing group 111’.  It is also a hazardous substance according to the criteria of the Australian Safety and Compensation Council.

The deceased was located in the tank in an unconcious (sic) state, and when retrieved from the tank did not regain conciousness. An expert analysis of the atmosphere inside the tank concluded that that (sic) there was a lethal concentration in all or part of the tank (10 litres of the dangerous good was used).  At the time of the incident a confined space permit was not issued, the deceased was not wearing respiratory protection, gloves or a harness, and there was no ‘spotter’ in place to supervise the latex removal works.

Further, there was a lack of training and supervision of employees in relation to the work procedures for confined space entry.”
